AI system complaints and feedback

Tell us about an AI system you used, a decision that affected you, or something that went wrong. You can stay anonymous unless you want a reply.

This channel supports the organisation's EU AI Act communication duties: people must be able to object, complain, and give feedback about AI systems. Data subject requests under the GDPR stay on the existing request form.

  • Article 50 — you must be able to know you are interacting with AI and to object or complain.
  • Article 72 — we collect and act on user feedback and complaints about system behaviour.
  • Article 26 — if a risk is found we inform the provider and the market surveillance authority.
  • Article 73 — serious incidents can be recorded and escalated here (this is not the personal-data breach register).
  • Article 86 — you can ask for an explanation of an individual AI decision.
  • Article 85 — you have the right to lodge a complaint with a market surveillance authority.

For access, rectification, erasure or other personal-data rights, use the data subject request form instead.

Your complaint right

You have the right under Article 85 of the EU AI Act to lodge a complaint with a market surveillance authority if you consider that an AI system or its use infringes the Act. Sending this form does not replace that right.
